In order for us to comply with the Florida Bar's strict rules of ethics, please provide the following information so that we may determine whether we have any conflict that would prevent us from providing our legal services to you.

While we will keep this information confidential and will not reveal it or use it to your disadvantage, you agree that by providing us with this information you do not create an attorney-client relationship with us. An attorney-client relationship will be created only if we expressly agree to provide you with legal services.

You also agree that providing us with this information will not limit our ability to provide legal services to other persons consistent with the rules of ethics. For example, if we are already representing your spouse and for that reason we decline to provide you with the legal services that you have requested, you agree that we may continue to represent your spouse. (See Florida Rule of Professional Conduct 4-1.18.)
